What are the responsibilities and job description for the RN MDS Coordinator position at River Run?
Responsibilities:
1. Coordinates, gathers data and completes Resident Assessment Instrument in an accurate and timely manner.
2. Implements individual plans of care with interdisciplinary approach in accordance with federal, state and local regulations and the established policies and procedures of Hillstone Healthcare. Updates care plans with all OBRA assessments, daily orders, and observed/recorded changes in care.
3. Transmits, or oversees this function, of MDS data adhering to federal and state guidelines, observes for trends in errors, and completes modifications/in-activations as needed.
4. Evaluates and re-evaluates as necessary resident needs and level of care by the standards and requirements of licensure and regulations and MDS requirements.
5. Coordinates efforts from all resources available, i.e., clients, staff, health care professionals and the care plan team to achieve optimal plan for each individual plan of care.
6. Participates as needed in Managed Care pre-certifications and re-certifications.
7. Attends and participates in individual resident quarterly care planning meetings as needed, quarterly QI/QA committee meetings and other committee meetings as deemed appropriate by the DON and/or Administrator.
8. Obtains CASPER reports, assists in analyzing data, and provides data to the IDT.
9. Assures that all assessments are completed in a timely manner by all departments, including interview portions, and provide education as needed on maintaining compliance.
10. Participates and assists in the overseeing of care planning and modifying of the care planning process as needed.
11. Monitor the quality and appropriateness of nursing care via review of the medical record during completion of the MDS and assist in the education of staff as needed.
12. Participates in the development, periodic review and evaluation of nursing policies and procedures.
13. Participates in surveys by authorized government agencies and maintains the 672/817.
14. Maintains privacy and confidentiality of records, conditions and other information relating to residents, other employees and the facility.
15. Attends and participates in staff on-going training and educational classes.
16. Coordinates with therapies -- Occupational, Physical and Speech – regarding resident rehabilitation, scheduling of OBRA/PPS/OMRA assessments, obtaining section GG/ Functional Assessment Data, screening monthly per the OBRA calendar, screening based on noted/observed changes, daily PPS meeting.
17. Monitors CMI both proactively and retroactively, ensures compliance, provides education as needed, completes audits as needed, participates in corporate reviews, follows Ohio, Federal and Hillstone policies, ensures the appropriate reimbursement to reflect care provided.
18. Other duties as assigned by the Director of Nursing and/or Administrator including, but not limited to, on call duty and other supervisory duties.
Qualifications
Education:
1. Graduation from an accredited nursing program.
2. Progressive experience in nursing practice and nursing leadership.
3. Current license to practice as a registered nurse in Ohio.
4. Experience in nursing practice including 2 years experience in Long term care nursing and a minimum of 1 year of MDS experience.
5. MDS Coordinator Certification preferred.
