Job Description:

We are seeking an outgoing, vibrant individual to join our dynamic team as a Social Services Director/Designee for our long-term care facility. The ideal candidate is one who is passionate about making a positive impact on the lives of our elderly residents and has a knack for fostering an environment of social engagement and fun. This role involves developing and implementing programs that cater to the unique needs of our residents, providing emotional support, and ensuring compliance with all relevant regulations.

Responsibilities:

• Collaborate with the medical director to develop and implement resident care policies that are consistent with current professional standards of practice.

• Assess residents' physical, intellectual, emotional, social, and spiritual needs, and tailor programs to meet these needs.

• Coordinate with Medicare-certified hospice services when necessary and maintain ongoing collaboration and communication processes.

• Conduct Significant Change in Status Assessment (SCSA) as required.

• Provide guidance and support to residents and their families, assisting them in navigating care plans, Medicaid eligibility, and other pertinent processes.

• Act as a liaison with State regulatory and informational agencies, resident advocacy groups, and other relevant entities.

• Address and resolve any resident complaints promptly, ensuring compliance with state and federal nursing facility regulations.

Qualifications:

·       Degree in a related field and/or relevant experience in long-term care settings.

·       Excellent communication and interpersonal skills.

·       Ability to work effectively as part of a multidisciplinary team.

·       Knowledge or willingness to learn about CMS regulations and guidelines.
