Summary:
- Graduate of an approved school of nursing, BSN preferred.
- Current registration or application thereof with current work permit with the Illinois Board of Examiners and the Illinois Dpeartment of Professional regulation and Education.
- Current CPR certification required.
Responsibilities:
- Acquisition of equipment and supplies, notification of pharmacy meds, short stay residents to ensure medications are ordered.
- A ctively participates in providing feedback, task force development and implementation of measures to improve and evaluate effectiveness of Quality Improvement Measures and delivery of services throughout the RHS continuum.
- A ctively participates in nursing department, town meetings, and Employee opinion surveys.
- A ssumes responsibility for obtaining continuing education relating to geriatric, federal, state and local regulations.
- C hecks CWF (Common Working File) and background checks are completed per policy prior to admission.
- C onfirms benefit coverage of insurance/confirms payer source prior to admission.
- C ommunicates information to MDS/IDT team to determine RUG categories, level of care, payment source.
- C omplete regulatory required criminal background checks on all residents accepted for admission.
- C ompletes initial physical admission assessment and documentation of plan of care.
- P rovides hand-off report to receiving nurse.
- E valuates and conducts resident/family education needs for diagnosis and discharge planning.
- C oordinate and implement referral/workup process and the entire admission process from time of referral to actual move in to ensure smooth transition during move-in process.
- E nsure accurate completion of required forms, executes and ensures contracts, ensures timely completion of all paperwork, DNR, Medicare certifications/recertifications.
- M aintains an ongoing audit of current records, charts, to ensure their completeness and compliance with regulations.
- P rovides information to referrals/referral sources who are unassigned to a MD who has privileges with RHS.
- R eports and follows up with Director of Nursing on a daily basis any new admissions, discharges, deaths, unusual or unsafe situations in existence.
- V erifies admission orders with primary care physician to ensure accurate/complete orders are in place prior to admission.
