Summary:

• Graduate of an approved school of nursing, BSN preferred.
• Minimum of one year experience as a professional nurse within the past 3 years or completion of a refresher course in the past year.
• Previous Home Care experience desirable.

  License or Certification Requirements:

• Current IL RN license
• CPR certification preferred
Valid driver's license and personal auto insurance required

Responsibilities:

• Accepts all calls made to the agency during the specified on-call hours.
• Accepts orders on new admissions, assigns qualified employee and/or visits the patient as indicated by patient need and physician order.
• Accepts referrals based upon agency admission criteria, staffing ability and geographic location of patient to be served.
• Administers direct nursing care to patients based on the physician's plan of treatment and the patient care plan in accordance with qualifications and age specific competency.
• Assessments, summaries, transfers and discharge documentation is completed and submitted as required according to agency policy and regulation.
• Assist each patient to reach his/her maximum potential.
• Communicates any changes in the plan of care to the employee and patient.
• Communicates positively and effectively with patients and families, employees, physicians, answering service and referral sources.
• Completes assignments to non-professional staff, based upon competency when indicated.
• Conducts the admission of the patient into the agency services through a comprehensive patient assessment which is completed on a timely basis.
• Cover On-Call Pager and Visits as needed for On-Call staff (call offs or scheduled time off).
• Develops and implements the patient care plan to best meet patient's needs, establishment of outcome goals, and actions taken to achieve these goals.
• Maintains a complete, accurate record of calls and submits a verbal and written report at the completion of the duty.
• Maintains at least an ongoing 75% rate of attendance at staff in-services/unit meetings and is compliant with organizational competency requirements.
• Maintains productivity requirements.
• Makes home visits on current patients when unanticipated needs occur or refers to qualified personnel in accordance with physician orders and/or plan of care.
• Obtains the necessary patient and physician information.
• Prepares a clinical note every visit with documentation submitted and validated according to policy.
• Recognizes spiritual and psychological needs of patients and their families when planning care.
Uses appropriate teaching techniques and interventions when providing new education or reinforcing previous teaching to the patient or family in the home setting.

Lifting of patients off their beds that are unable to move independently is done with assistive lifting equipment. Horizontal movement of patients to and from carts/beds who are unable to move independently is done with assistive lifting equipment. Lifting equipment - carry 25'-150' waist high (IV bags, patient trays). Carry charts 25'-150' waist high. Filled laundry bags are filled half full so as not to weigh over 35 lbs. Carry fire extinguishers up or down 3-4 flights of stairs and several yards in an emergency.
