Wage: LPN: $17.00-$27.00 RN: $ 26.00-$40.00 hourly; depending on number of years of transferrable experience and internal equity**
MATERNAL CHILD HEALTH REGISTERED NURSE SUMMARY
Provides nurse case management services to assigned clients who are high risk pregnant women and children with special healthcare needs. The principal duties are performed in client’s homes, as well as in office, community settings or telehealth.
KIDSFIRST REGISTERED NURSE CASE MANAGER SUMMARY
KidsFirst Program strives to provide coordinated care to work with all systems that impact children in foster care including but not limited to the foster care system, physical and behavioral healthcare, and families or caregivers to ensure the healthcare needs of children in foster care are met.
ESSENTIAL SKILLS AND RESPONSIBILITIES include the following. Other duties may be assigned.
- *Maternal Child Health Home Visitor
- Provides comprehensive nursing assessment, health care planning and coordination of medical care for pregnant women and children with special healthcare needs lead by a Registered Nurse.
- Assesses and determines the physical, social, and environmental needs of clients.
- Provides information, support, advice and appropriate referrals relating to individual high risk pregnant women and children with special healthcare needs.
- Participates in a manner which ensures high standards of customer service delivery in response to client and community partners’ needs and expectations.
- Provides an innovative approach to service delivery that will encourage access to health care and community resources.
- Provides a focus on prevention, early detection and intervention of the health and well-being concerns of high risk pregnant women and children with special healthcare needs through an interdisciplinary and integrated service response.
- Recognizes and facilitates access to families of culturally and linguistically diverse backgrounds.
- Acts as an advocate for pregnant women and children in the community.
- Participates in relevant networks and outreach programs concerning pregnant women and young children as requested.
- Informs the medical provider of any changes in patient’s condition or needs and coordinates any modifications in treatment with other appropriate personnel.
- Maintains appropriate electronic records.
- Participates in in-service programs, orientations, and community presentations.
- Makes appropriate referrals within the agency and other community resources.
- Attends meetings, conferences, workshops, and training sessions for ongoing professional development.
KidsFirst Home Visitor
- Provides comprehensive nursing assessment, health care planning and coordination of medical care for children in foster care lead by a Registered Nurse.
- Provides medical case management and health education services to clients and their foster families in their homes or location of choice.
- Provides on-going emotional support and acts as an advocate for client with other community support services and organizations.
- Assesses and determines the physical, social, and environmental needs of clients.
- Develops a comprehensive plan of care with other medical team members including primary care providers, dental providers, medical specialists, care coordinators, and community resources lead by a Registered Nurse.
- Completes initial comprehensive medical records review and in home assessment to determine medical needs of each client lead by a Registered Nurse.
- Completes ongoing assessments and case management activities for the duration of participation in KidsFirst via home or phone visits.
- If indicated, attempts and/or completes home or phone visit upon return to biological family during trial home visit period.
- Complete age appropriate screenings as indicated including ASQ, ASQ-SE, depression, tobacco and substance abuse.
- Informs the primary medical provider of any changes in the child’s condition or needs and coordinates any modifications in treatment with other appropriate personnel.
- Maintains appropriate electronic records.
- Provides electronic records from each visit to Child and Family Services Division in Yellowstone County.
- Participates in in-service programs, orientations, and community presentations.
- Makes appropriate referrals within the agency and other community resources.
- Provides support and information to foster parents in regards to the client’s medical status and care.
- Meets with supervisor on regular basis to staff client needs.
- Attends meetings, trainings and workshops for ongoing professional development.
- Responds to client and family questions and comments in a courteous and timely manner.
- Participates in program quality improvement projects.
- Participates in emergency preparedness activities as assigned.

EDUCATION AND EXPERIENCE
- Registered or Licensed Practical Nurse Program
- Experience in maternal child, pediatric nursing or public health preferred
- Skills in electronic medical records
- *CERTIFICATES, LICENSES, REGISTRATIONS:
- Current license as a Registered or Licensed Practical Nurse in the State of Montana;
- Valid State of Montana Driver’s License in good standing.
- Required to obtain emergency preparedness on the job training at a minimum of ICS 100, 200, and 700.
