Job Summary:
The SED Case Manager facilitates coordination, communication and collaboration on behalf of the clients/families of children who have a serious emotional disability (SED) in order to achieve goals and maximize outcomes.
Minimum Qualifications:
- Bachelor's degree in a behavioral science from an accredited institution. Behavioral sciences include Psychology, Social Work, Sociology, Human Services, Special Education, Family Studies, Counseling, and Nursing.
- One (1) year relevant paid experience in children's services or case management after Bachelor's degree in a behavioral science is completed. A Master's degree in a behavioral science can substitute for the one year of experience.
- Valid Driver's license and ability to provide safe and reliable transportation.
- Good oral and written communication skills.
- Adequate hearing ability.
- Understanding of family/individual/group dynamics.
- Understanding of agency policies and procedures.
- All case managers are required to complete and pass the Safe Crisis Management 2-day class prior to independent functioning. The SCM training must be re-certified annually. Case Managers must possess the physical ability to perform SCM techniques.
- State Case Management certification within six (6) months of employment.
- Case managers may be required to provide services for up to 25 children and their families.
