SUMMARY
The Community Outreach Referral Coordinator must be knowledgeable of all services offered within Rochester Regional Health Chemical Dependency in order to support the mission to connect patients to the best level of care and support patients in reaching their goals for the treatment of substance use. The employee must participate as a contributing member of an interdisciplinary team by providing superior customer service and modeling the Brand Promise and Core Values at all times. The Community Outreach Referral Coordinator is also responsible for working with various internal and external partners to connect patients as quickly and safely as possible to one of our three inpatient units, or facilitating a connection to a different level of care if deemed the most appropriate. The Community Outreach Referral Coordinator must possess exceptional communication and organization skills and have work experience within customer service and/or administrative assistance.

RESPONSIBILITIES
o Must be able to communicate with various disciplines including: medical staff, nursing staff, clinical staff, psychiatric staff, program managers, and external community partners when necessary.
o Identify potential new relationships with community partners and work to build these relationships.
o Conduct regular meetings with internal and external partners who refer patients to RRH Chemical Dependency inpatient rehabilitation in order to build and maintain strong relationships.
o Responsible for tracking outcomes related to referral volume changes following meetings with internal and external partners.
o Provide excellent customer service when speaking with prospective or current patients as well as with both internal and external community partners.
o Assist with scheduling and assigning of appointments in order to connect patients to evaluators to assess for the most appropriate level of care based on patient choice.
o Conduct group presentations and educational programs as needed, which may include local travel to community partners.
o Participate in professional development and education.
o Provide case management and communication with referral sources.
o Attend and participate in team meetings and is a team player.
o Identify potential new relationships with community partners and work to build these relationships.
o Other responsibilities as assigned.
