SUMMARY
The goal of RS Eden Support Services is to improve individual and community safety, holistic wellness, recovery, increase skills and income, and housing stability. In Supportive House, RS Eden takes a trauma sensitive, Housing First, and recovery focused approach The Housing Case Manager does this by meeting with clients on individual levels as well as promotes community growth and wellness through group leadership with particular sensitivity to the complexities of mental health, substance use, and trauma.
Case Managers will meet with clients to work in individual goal plans on a consistent basis and support them in changes in their goals. Creativity is needed as Case Managers work with clients to address complex barriers when providing services, referrals, and working on case plans. They must be able to balance long-range goals of clients with step-by-step goal planning as well as program and agency needs and requirements. Case Managers act as professional role models when engaging with both clients, coworkers, and collaterals to reinforce the mission of RS Eden and support the communities in healthy growth.
The Case Manager position requires the ability to navigate complex situations with positivity and creativity. To work in Supportive Housing a case manager must possess flexibility, critical thinking skills, communication skills, and the ability to react effectively to high-intensity and crisis situations and an ever-changing environment.
RESPONSIBILITIES
• Provide Person Centered Case Management, Client Skills groups and Community Supports.
• Respond to clients who experience critical challenges impacting housing stability, health, and/or safety.
• Develop and maintain a working knowledge of available community resources.
• Act as a role model for clients by exhibiting productive behavior and conducting service interactions with a high level of professionalism and boundaries.
• Maintain electronic health records in accordance with the expectations for timely and thorough billing for services rendered.
• Safeguard personally identifiable health information (PHI) in accordance with duties and current agency HIPAA notification statement and standards.
• Participate in and successfully complete training as required or directed.
• Flexible schedule to meet case load need
• Represent RS EDEN to the community at large via positive relationships with area residents, business neighbors, donors, and other professionals interested in RS EDEN programs
• Non-violent and non-contact crisis de-escalation
• Other duties as assigned.
QUALIFICATIONS/REQUIREMENTS
Required:
- BS/BA in a field of study in health or human services or the equivalent training experience and/or lived experience is required.
One year of professional experience in Human Services is preferred. - Must possess intermediate computer skills which include a moderate understanding of operating systems, word processing, spreadsheets, databases required.
- Excellent verbal and written communication skills required.
- Ability to relate to people from diverse backgrounds with multiple individual and systemic barriers to success required.
- An understanding of substance use recovery and/or severe mental health and wellness
- The capability of working alone with very little supervision.
- Must be able to travel within the cities of St. Paul and Minneapolis when necessary complete job duties.
