The Homeless Services Case Manager manages a caseload of clients who are homeless or at risk of homelessness, diagnosed with a mental health issue, behavioral disorder, or pervasive developmental disorder. The Case Manager provides community case management services to clients who meet eligibility requirements. Services include intake and assessment, individual service planning, crisis and support planning, coordination and advocacy through linking clients with resources, monitoring and evaluation. The Case Manager will establish trusting relationships and provide direct services with a goal of teaching clients to develop independent living skills and the skills needed for self-sufficiency and stability.

Education Educational and experience requirements include:
*Bachelor’s Degree from an accredited four (4) year institution of higher learning with a specialization in psychology, mental health and human services, behavioral health, behavioral sciences, social work, child development, special education, counseling, rehabilitation, sociology, nursing or closely related field; or
* Bachelor’s degree from an accredited four (4) year institution of higher learning in an unrelated field and at least one (1) year of full time equivalent relevant human services experience.
