DESCRIPTION:
Coordinator Support Services are assigned to serve as the primary point of contact with families to facilitate arrange and manage service delivery and supports for families with children eligible to receive early intervention services. Some job function includes: Coordinating the performance of evaluations and assessments after initial-eligibility is established and annually. Conducting, as appropriate, a Family Directed Assessment; Facilitating and/or participating in the evaluation, periodic and annual review-and development of Individualized Family Service Plans (IFSPs); Assisting families in identifying available service providers/practitioners to-initiate timely services within 30 days of the parent consent to the IFSP.
REQUIREMENTS:
Bachelor s Degree in Education, Health, Psychology, Social Work, Social Science or other Human Service Field. A valid, unrestricted NJ Driver s License required. Must have own vehicle and availability to Travel; highly organized; minimum of 1 year professional experience working with children between 1 and 5.
