Job Description
Responsible for the provision of pharmaceutical care to all patient age groups (including geriatric, adult, adolescent, pediatric and neonatal patients). They are responsible for the preparation, dispensing and monitoring of both the prescribing and effectiveness of all medications and IV admixtures. They exercise considerable independent decision-making.

To qualify you must Completion of course at an accredited School of Pharmacy, with a degree of Bachelor of Science in Pharmacy. Successful completion of all orientations. Hospital Pharmacy experience desirable. Registration in New Jersey as a Pharmacist. Current New Jersey licensure required. Familiarity with all phases of Hospital Pharmacy. Working knowledge of health care matters. Understanding of role of Pharmacy Department within the Hospital, its interrelationships with other departments and functions. Utilizes the medication screening process to insure medication orders are complete, appropriate, legible, within legal parameters and prioritized. Evaluates each medication order for formulary status, appropriate dose, dosing interval, allergy, drug/drug interaction, and drug/disease state interaction. Oversees preparing of parental admixtures, dispensing, and compounding of medications. Contacts physician directly for orders requiring intervention and maintains appropriate entries in intervention log. Notifies nurse of any changes/modifications of medications prescribed. Displays self initiative in anticipating and resolving daily work issues. Supervises technical staff and volunteers to ensure accuracy and compliance with policies and procedures. Provides direct supervision of all non-pharmacist personnel, to include checking of computations and medication cassettes prior to delivery. Assures proper labeling of any open or reconstituted vials of medication with initials, date, amount of diluent, and final concentration.Identification is complete, clear and per protocol. Dispenses medication is properly packaged in unit of use packaging. Evaluates and reports Adverse Drug Reactions and Adverse Medication Events. The staff member participates in the interdisciplinary patient teaching process. Assumes role of drug information provider to all members of the health care team specific to patient populations and age groups. Participates in activities, committees, projects and teams. Follows proper safety and infection control procedures and maintains a safe work environment. Participates in the safety and infection control management programs. Participates in ongoing education and training related to organization and department specific requirements. Monitors specific medication prescribing in accordance with established drug usage criteria, prior to dispensing. Independently conducts floor inspections at specified intervals. Properly complies with all national patient safety goals/risk reduction processes and associated policies and procedures as applicable to medication management and the Pharmacy setting. Utilizes the medication screening process to insure medication orders are complete, appropriate, within legal parameters and prioritized. Notifies nurse of any changes/modifications of medications prescribed. Participates in ongoing education and training related to organization and department specific requirements. Monitors specific medication prescribing in accordance with established drug usage criteria, prior to dispensing. Evaluates and reports Adverse Drug Reactions. Enhances professional growth and development through participation in educational programs, current literature, workshops,in service meetings, etc. Maintains the confidentiality, security and integrity for the management of associate/patient information according to policy and procedure. Participates in medication error and adverse drug reaction reporting programs. Assists with orientation and training of new employees, students, volunteers and medical staff.
