Job Description:

The successful candidate for the Case Manager will serve as a member of a team of mental health professionals, who provide direct and indirect clinical services to the University community out of the wellness center. This individual will provide crisis counseling, consultation services, and case management services to students in the campus community. Furthermore, this individual will manage referrals from within the university community, provide outreach to students, facilitate off-campus referrals, provide advocacy on behalf of students, and coordinate with community partners to assist students in accessing care based on their treatment needs.

Responsibilities Include:

  • Provide and facilitate case management and coordination of care for students requiring referrals off campus
  • Provide individual counseling and/or same-day crisis counseling, intervention, and safety planning
  • Oversee all Pio-connect and other campus referrals, outreach, and oversight
  • Manage and expand upon the counseling center's referral network for providers by maintaining existing relationships and fostering new relationships in the community, including but not limited to specialized care centers, hospitals, and IOP/PHP programs.
  • Build relationships with on campus departments to support student wellness
  • Assist staff with referrals for students in need of care outside of the counseling center
  • Assist students in accessing care and treatment in the process of leaving or returning from a medical leave of absence
  • Provide case management training workshop to counseling staff and larger university departments as requested
  • Participate in on-call rotation
  • Engage in outreach opportunities
  • Staying clinically up-to-date by attending continuing education workshops and conferences
  • Keep timely and updated case notes consistent with the counseling center policy
  • Engage in supervision
  • Work as part of a treatment team
  • Attend University functions/specific events/meetings.
  • If licensed, supervise master's level trainee(s) in Social Work or closely related field
  • Attend all counseling center staff meetings
  • Some weekend and evening hours.
  • Other duties as assigned



Requirements:

  • Master's degree in social work or closely related discipline with at least 1-2 years of relevant experience
  • Have a state license or obtain one within a two-year period from date of employment
  • Experience working on a college campus preferred
  • Able to relate to college students
  • Possess awareness of multicultural and diversity issues and cultural humility
  • Abilities and attributes that enable effective representation of mental health issues in the university setting
  • Able to work collaboratively as a member of a team
  • This is a 10-month position.
