Job Overview

Safari Healthcare Associates is seeking a dedicated and compassionate Social Worker to join our Home Health Care team. As a Social Worker, you will be responsible for providing support and assistance; including effective interventions, goal setting with clients, counseling and appropriate referrals to individuals, groups, and families. The ideal candidate will have experience in crisis management, behavioral health, and child welfare. Emphasis of duties may vary depending upon area of assignment.

Duties

• Works in close collaboration and communication with physicians and other members of the health care team in order to minimize duplication of effort, promote teamwork, and ensure seamless coordination of services within and between teams and across campuses.
• Assists with patient evaluation and treatment to further their understanding of significant social and emotional factors underlying a patient’s health problem(s). Actively participates in care conferences as appropriate.
• Mobilizes patient/significant others’ to utilize their own strength to make necessary lifestyle and behavior changes. Acts as a patient advocate to ensure their desires are recognized and followed to the greatest extent possible. Identifies and responds to any situations representing risk to the patient, their family, the organization and/or the community.
• Partners with community agencies to coordinate care across the continuum to offer options, tools, and resources to allow patients the opportunity to achieve his or her desired goals.

Experience:
- Bachelor's or Master's degree in Social Work or related field
- Valid state licensure or certification as a Social Worker
- Experience in Medicare billing and documentation
- Knowledge of crisis management techniques and interventions
- Familiarity with behavioral health issues and treatment modalities
- Strong understanding of child welfare policies and procedures
- Ability to conduct addiction counseling and provide support for substance abuse issues
- Proficiency in motivational interviewing techniques
