Job Summary: A Bachelor's prepared (BSW) Social Worker assists with securing safe discharge placement of patients through helping patients/families identify discharge needs, verify health benefits, provide discharge planning for appropriate level of care post discharge including facilitating placement/transfer to another facility and securing necessary medications for discharge. Maintains an accurate record of patient/family contacts and documents pertinent information in the medical record.
Education: Bachelor's degree in social work or related field.
Licensure, Registration and/or Certification: None.
Work Experience: 6 months of related experience.
Knowledge, Skills and Abilities: Excellent organizational, interpersonal, and communication skills. Knowledge of community resources. Ability to utilize computer resources and have basic computer skills.
Safety Sensitive: Under the Oklahoma Medical Marijuana Use and Patient Protection Act (OMMA), safety sensitive position is defined as any job that includes tasks or duties that the employer believes could affect the safety and health of the employee performing the task or others. This position has been identified as a safety sensitive position. This statute allows employers to lawfully refuse to hire applicants for safety-sensitive jobs or to discipline or discharge employees who work in safety- sensitive jobs if they test positive for marijuana, even if they have a valid license to use medical marijuana.
