Part-time, Full-time, Every other Weekend required

Essential Functions :

• Provides and / or assists with providing, while promoting optimum resident independence, activities of daily living, including but not limited to, bathing, dressing, grooming, transfer and ambulation, toileting, eating and use of speech, language or other functional communication systems for residents.
• Assists with basic therapeutic care not requiring nursing judgment, such as obtaining heights and weights and measuring vital signs.
• Reports any change in resident condition immediately to appropriate person to assure quality and continuity of care and adjustments to plan or care as needed.
• Personally follows appropriate safety policies and procedures of facility and regulatory agencies, reports all broken and malfunctioning equipment immediately, follows disaster plans during participation in preparedness drills and actual emergencies and maintains a neat orderly work environment.
• Personally follows appropriate infection and control policies and procedures of facility and regulatory agencies.
• Prioritizes assignments to complete care for residents in allotted time making adjustments throughout shift to meet immediate need of resident when necessary and assuring prompt answering of call lights.
• Assists with admission of residents, i.e., obtaining admission heights and weights; seeing that personal items are marked and properly recorded;

and seeing that personal items are properly put away.

• Assists with transfer or discharge of residents assuring proper care of personal items to prevent loss or breakage.
• Completes neatly and accurately required documentation according to Nursing Home standards and policies.
• Demonstrates good communication and interaction skills when dealing with supervisors, co-workers, residents, resident's families and other departments.
• Assists in teaching the resident and / or family, as directed by immediate supervisor, the necessary skills to allow the resident to return the his / her home if applicable.
• Participates in performance improvement / quality assurance activities as directed.
• Attends mandatory and non-mandatory in-services and staff meetings.
• Maintains a strict level of confidentiality concerning residents.
• Receives report from immediate supervisor and utilizes assignment sheets and kardexes to maintain knowledge of current resident status.
• Assure delivery of optimum resident care by being to work on time, as scheduled and / or providing appropriate advance notification.
• Performs other duties and special projects as assigned by immediate supervisor.
