Summary of Responsibilities:

Under the general direction of the C&Y Specialized Services Supervisor, this person provides educational programs on home management, life skills and parenting to individuals/families receiving Children and Youth Case management services. This person assesses homes upon each visit and make recommendations as needed. Duties are conducted in accordance with all existing policies, procedures and regulations and in accord with the Mission and Values of the Company. This position has been designated to be a part of the County's CYS workforce for purposes exclusive to the proper handling of consumer information as is required by SAM privacy policies.

Essential Functions:

• Provides home visits

• Assists in family's goals

• Assumes full responsibility for the operation of the home and care of the children during the absence and incapacity of the parents

• Assists families to function cohesively

• Develops/monitors plans

• Assists in identifying community resources
  
• Provides supervised visitation
  
• Provides client transportation
  
• Attends/testifies at Court Hearings
  
• Completes case notes
  
• Completes required trainings
  
• Attends meetings

Education and Experience:

High school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years direct care experience in public or private human services. Or high school diploma and five years of direct care experience in public or private human services.

Employment Requirements:

Automobile and Valid Insurance, Valid Pennsylvania Driver's License and Acceptable Motor Vehicle Record Check, Act 33 and Act 34 Clearances, FBI Fingerprint Clearance, Social Security Card, Immigration and Naturalization (I-9), Drug Screening Clearance

Working Conditions:

In office work performing concurrent varied tasks Travel throughout the assigned region may occur and may be in adverse weather conditions.
