- Updates and ensures accuracy of social service notes, social histories, social service assessments, move out documentation, and additional related information.
- Assists in developing and maintaining written social service related policies and procedures.
- Interprets the social services policies and procedures to employees, residents, visitors, government agencies, etc., as necessary.
- Participates in facility survey process and develops plan of correction for social service deficiencies.
- Participates in resident assessment and documentation process including MDS, care planning, social history/assessment, and on-going evaluation of resident psychosocial needs.
- Participates in care conferences with residents.
- Maintains contact with referral agencies and internal waiting list to ensure openings are filled appropriately and timely.
- Plans and participates in move-out meetings. Assists residents and families with planning and coordination, as appropriate.
- Addresses resident concerns in a timely manner. Takes appropriate action to resolve the concern(s) and/or refer to the appropriate discipline. Documents resident concern and resolution process.
- Responsible for participating in different meetings and committees throughout the organization. This includes leadership, quality, safety, Vulnerable Adult, etc.
- Reports vulnerable adult incidents to the appropriate agency and participates in investigations.
- Assists residents and/or families with Medical Assistance applications or other forms/applications necessary to secure financial reimbursement to the organization.
- Continually seeks opportunities to meet resident’s psychosocial needs through both care planned and spontaneous activities.
Requirements:
Current Minnesota Social Work license.
Minimum 3 years of social work experience in Long Term Care.
