Job Summary: Under the direct supervision of the Director of Patient Care Services, provides social work services, utilizing a comprehensive base of psychological theory. Approaches care in a systematic manner in accordance with the standards for home care and home health social work. Documents in a timely, accurate manner to ensure appropriate billing submissions. Demonstrates initiative in individual professional development. Completes all education required by Samaritan Home Health and for licensure/recertification. Incorporates and educates patients, family and caregivers in treatment as appropriate. Works collaboratively with in-house clinicians.

Minimum Requirements:
· Has completed a masters or doctoral degree in social work or related field.
· Minimum of one (1) year experience in a health care setting or equivalent experience. Home health experience preferred.
· CPR certification
· Demonstrates strong interpersonal skills.
· Demonstrates knowledge of resources available in the community.
· Licensed driver with automobile insurance in accordance with state/Agency requirements. Reliable, registered and insured transportation and demonstrated good driving record.
· Strong interpersonal skills. Ability to communicate effectively in writing and verbally with people who have varied backgrounds.

Job Type: Contract
Pay: From $35.00 per hour
Expected hours: 2 – 40 per week
