POSITION TITLE: SERVICE COORDINATOR

Non-Exempt Salary (37.5 hours per week)

Location: Watsonville, CA

Job Summary: While maintaining good public relations and implementing the goals and vision of the regional center, this position is responsible for providing and coordinating services to consumers in accordance with the provisions of the Lanterman Developmental Disabilities Services Act, and policies established by the San Andreas Regional Center Board of Directors.

Essential Job Functions:

  • Provide enhanced service coordination to persons with developmental disabilities and their families
  • Manage a 1:40 caseload ratio of consumers currently served by SARC with the lowest per capita and/or no POS expenditure
  • Utilize a Person Centered Plan (PCP) approach to the Individual Program Plan (IPP) Process
  • Identify barriers in accessing services and supports to enhance relationship between
  • consumer/family
  • Identify areas of unmet needs
  • Empower individuals to partner with SARC and other community agencies and to assist in identification of new resources that are culturally relevant to our community.
  • Work in collaboration with the PCP Specialists, Cultural Specialist, and other SARC Departments to identify innovative approaches to service coordination
  • Exercise PCP strategies responsible for the development and implementation of the consumer's IPP, progress reports, the consumer's annual reviews, and other reports as

applicable A Bachelor’s Degree in social work, rehabilitation counseling or a related field from an accredited college or a Bachelor's Degree in Social Work plus four years of experience. The specific field of study for the Bachelor's Degree and the post Bachelor's years of experience may be waived only by the Executive Director if needed by the agency to meet a special need, e.g. services to specific cultural or monolingual groups.

  • Provide follow-up, reassessments, and periodic review of consumer's progress and program effectiveness
  • Uphold the agency mission, philosophy and core values
  • Assist in locating and developing natural supports
  • Knowledgeable of current state and federal regulations, agency policy and procedure and guidelines related to consumer services, including Title 17 and Title 22
  • Initiate the purchase of vendored and generic services and monitor service delivery
  • Advocate on social, legal, education, mental health and/or forensics issues related to the consumer
  • Participate in outreach and represent the agency in community meetings and events
  • Conduct public presentations regarding regional center services and activities
  • Serve on intra-agency and/or inter-agency committees
  • Participate in data gathering and analysis activities
  • Additional duties as assigned as deemed appropriate.
  • Maintain meticulous records/documentation
  • Willing to travel to all four counties served by SARC
  • Perform other related tasks as requiredEducation/Experience:

Special Skills and Knowledge: Knowledge of principles, objectives and techniques of social work; individual and group behavior, human growth, development of family relationships; medical-psycho-socio-economic factors affecting individuals and families; techniques of intervention, referral and follow-up to community resources and facilities and various community organizations, and how to intervene constructively on the client's behalf. Ability to relate in a constructive and enabling manner to a wide variety of individuals and organizations; ability to relate in a positive manner with supervisor, co-workers, and others in the agency and community; establish and maintain rapport with clients and their families; prepare and present oral and written reports; maintain case records in a clear, concise, appropriate and timely manner.

Other: Use of an automobile, possession of a valid driver's license and automobile liability insurance for the minimum amount prescribed by law, OR ability to provide for independent transportation.
