Job Summary
Plans, directs, and manages the clinical activities of a specialty program, and provides specialty care either directly or indirectly to patients. Maintains and monitors the program database and analyzes the data. Serves as a liaison between nursing staff, medical staff, pharmacy staff and other healthcare professionals, patients and their families. Additional responsibilities include the following:
-Monitor daily readmission rate, analyze readmission data for any trends leading to readmissions.
-Collaborate with clinical teams to ensure high risk discharges have the following components:
- Structured needs assessment ICM
- Social determinants of health ICM
- Medication reconciliation
- Patient education during their stay and promotion of self-management
- Anticipatory post-discharge guidance and contingency plan
- Facilitated follow up
-Coordinate with Transitions of Care team regarding follow up on high-risk readmissions to address issues related to symptoms, medications, and appointments.
-Collaborate with ECC staff to identify possible discharges that would prevent readmissions.
-Coordinate with clinics to arrange prompt follow up for ECC discharges and inpatient discharges.
-Assist with educating staff, patients, and caregivers regarding the importance of timely and appropriate follow up and mediation adherence.
-Monitor discharges with high-cost medications and collaborate with pharmacy and clinician on other affordable options.
-Collaborate to ensure patients are connected to community resources.
-Monitor CMS readmissions to prevent penalties from Hospital Readmission Reduction program.
Required Qualifications
- Require two (2) years of satisfactory RN experience; preferably in a designated specialty.
Preferred Qualifications
- Prefer certification in a specialty area.
- Prefer membership in a professional organization associated with specialty.
- Prefer program management, teaching, consulting, and outcome management experience.
Mandatory Education
BSN: Bachelor of Science Nursing
Required License and Certs
FL RN: FL Registered Nurse License
Hours: Monday-Friday, either 8:00am-4:30pm or 7:30am-4:00pm, flexible scheduling, no weekends required.
