Job Description: As a Behavior Health Case Manager with us, you will have the opportunity to be involved in the coordination of care and services for patients admitted to West Pines, Intermountain Health who are identified as needing assistance or meeting Care Management criteria. As a Social Work Case Manager – Behavioral Health, you need to know how to: Coordinate the care and services for patients identified as needing assistance or meeting care management criteria. Assess the patient’s prior level of functioning, access to and/or use of community resources and available support systems. Identify appropriate admission and continued stay issues. Attend, facilitate, and participate in rounds and case conferences. Conduct comprehensive behavioral health assessments for patients on mental health hold in the ED and on medical floors when ordered by a physician. Participate in in multidisciplinary care planning meetings. Communicate and collaborate with medical, interdisciplinary team staff, patient, family, insurance companies, and social agencies to provide quality discharge plan to appropriate, safe, level of care. Provide community resources for psychosocial, financial, and educational needs of patient and family. Your experience. We hire people, not resumes. But we also expect excellence, which is why we require: Master’s Degree in Social Work (MSW) from an accredited program Successful completion of Licensed Clinical Social Worker (LCSW) license exam and current clinical supervision Current LCSW license or the ability to obtain within three (3) years of hire Minimum of two (2) years of clinical experience Preferred: Current LCSW license At least two (2) years of case management experience in an acute care setting Your next move.
