We are seeking a compassionate, motivated Clinical Social Worker with a Mental Health specialty to join our acute care clinic at Scripps Mercy Hospital. This is a specialty care clinic providing care to our high-risk patients. You will assist patients and families dealing with personal and environmental problems which predispose them to illness, or which interfere with their obtaining maximum benefits from medical care. You will also
· Work with the RN Care Managers to provide the best patient care for a safe and timely discharge and
· perform high risk screenings and prioritizes consultations from hospital personnel appropriately
· conduct clinical assessments of patient/family to establish intervention strategies and safe discharge plans
· provide consultation in crisis interventions as well as counseling and advocacy services to patients who are coping with acute phases of illnesses
· participate in certification hearings for involuntary detainment
