What are the responsibilities and job description for the Housing Case Manager position at SEARCH Homeless Services?
Provides intensive case management services to chronically homeless adults in permanent supportive housing or rapid re-housing models. We have openings in several teams presently. Fulfils SEARCH’s mission to provide hope, create opportunity, and transform lives. Screens and assesses clients for eligibility according to grant and program requirements; enrolls eligible clients into program services. Provides intensive case management to chronically homeless adults using the Transtheoretical Model, Motivational Interviewing, and other evidence-based practices as appropriate. Builds network of referrals for supportive services for variety of needs related to exiting homelessness and actively coordinates referrals and care with other treatment and service providers in the community. Collaborates closely in team environment to coordinate care, provide social activities, and facilitate groups. Assists clients in obtaining mainstream benefits. Provides crisis services as appropriate to program clients. Maintains complete and accurate electronic records and paper files on each client served; follows agency standards for compliance and timeliness. Develops professional skills through participation in agency training and committees. Other related duties as assigned. Master’s Degree in Social Work or related field helpful. Bachelor’s Degree in Psychology or related field acceptable. Prior professional or internship experience with dual diagnoses populations ideal. Bilingual-Spanish is a plus.

Pay: $40,000.00 - $48,000.00 per year
