Job Title/Position: Director of Patient Care Services

POSITION QUALIFICATIONS

1. Registered Nurse with current licensure to practice professional nursing in the State of California.

2. An associate degree in nursing from an accredited program by the National League for Nursing. Bachelor’s degree in health care preferred.

3. Three years of management experience withing the last five years in a home health or related health care organization, at least one year of which was a supervisory or administrative capacity.

4. A registered nurse with four years’ experience within the last 5 years in a home health agency, primary care clinic or health facility, at least one year of which was in a supervisory or administrative capacity.

POSITION SUMMARY

Second Wind Home Health, a leader in healthcare excellence, is seeking an experienced and dedicated Director of Nursing to join our team. As a DON, you will play a pivotal role in providing clinical oversight of our skilled nursing operation. The purpose of your job position is to manage, develop and direct the overall operation of the nursing department in accordance with current federal, state, and local standards that govern the facility, and as directed by the CEO.

The Director of Nursing (DON) will treat each patient with kindness, dignity, and respect. The DON will maintain a positive and calm disposition. Communicate clearly, diplomatically, and effectively when speaking to patients, family members, caregivers, visitors, and staff.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES

1. Coordinates and oversees all direct and indirect patient services provided by clinical organization personnel.

2. Provides guidance and counseling to coordinators and Clinical Care Staff to assist them in continually improving all aspects of home health care services, provided through organization personnel.

3. Assist Clinical Staff in managing clinical teams and planning.

1. Provides help in assessment, planning, and implementation and evaluation of patient family/caregiver care to all clinical personnel as indicated.

2. Evaluates annual performance reviews of Clinical Care Staff.

3. Oversees the maintenance of patient clinical records, statistics, reports, and records for purposes of evaluation and reporting of organization activities.

4. Assures proper maintenance of clinical records in compliance with local, state, and federal laws and regulations.

5. Assures compliance with all local, state, and federal laws regarding licensure and certification of organization personnel and maintains compliance to the Joint Commission Home Health standards.

WORKING CONDITIONS AND SAFETY

· Demonstrates ability to supervise and direct professional administrative personnel.

· Knowledge and expertise in clinical decision-making and Quality Management review and evaluation.

· Must be a licensed driver with an automobile that is insured in accordance with state and/or Organization requirements and in in good working order.

· Has excellent observation, verbal, and written communication skills.

· Knowledge of business management, governmental regulations, and Joint Commission standards.

· Must possess mobility to work in a standard home healthcare setting and use standard medical equipment, including a computer, vision to read printed materials and a computer screen; and hearing and speech to communicate in person or over the telephone.

· This is primarily a sedentary Home Care Services classification although standing in homecare settings and walking between homecare settings are required.

· Finger dexterity is needed to access, enter, and retrieve data using a computer keyboard or calculator and to operate standard medical equipment.

· Positions in this classification frequently bend, stoop, kneel, reach, push and pull. Frequent driving is required in this position with inclement weather conditions.

· Outside temperatures conditions can vary depending on the season, such as Summer, Winter, and Fall.

· General Healthcare Resources considers all forms of nursing to fall into the medium category for physical demands, meaning that the job requires the ability to occasionally exert between 20 and 50 pounds of force and the ability to frequently exert between 10 and 25 pounds of force.

· Prolonged or considerable walking or standing. Able to lift, position, or transfer patients.

· Employees work in a home setting environment with moderate noise levels, controlled and non-controlled temperature conditions, and no direct exposure to hazardous physical substances.

Job Type: Part-time

Pay: $2,164.00 - $3,500.00 per week

Expected hours: 8 – 24 per week
