ABOUT THE TEAM

The Supported Living (SL) team provides support for over 50 men and women, up to 24 hours a day, seven days a week if needed. Most people supported in the SEEC SL department live with a roommate in an apartment or townhouse in Montgomery County. Supports range from drop-in assistance with grocery shopping and bill paying to 24-hour support for those with more significant needs.
SEEC staff work with each person to ensure their home represents how they want to live.

IN THIS ROLE, YOU'LL:

1. Support a person through a rigorous person-centered planning process to determine their personal goals and desires related to living in their community.

2. Coordinate services from Social Security Administration, Service Coordination and Social Services for people. Complete necessary paperwork to maintain Medicaid & social security benefits & other related forms in collaboration with SEEC’s benefit specialist.

3. Advocate for the person’s right to live, work, and thrive as they choose. This involves learning about rights and responsibilities, supporting independent decision-making and problem solving, as well as modeling and practicing conflict resolution.

4. Support the person’s membership in communities large and small—from friendship and family circles to citizenship at the local, state, and national level. Facilitate learning about what it means to be in reciprocal relationships.

5. Meticulous data management. This ensures that each person is receiving the appropriate services as their individual plan dictates and billed accordingly.

PHYSICAL ENVIRONMENT & WORKING CONDITIONS:

  • Travel throughout the county in all types of weather.
  • This a remote position and will be primarily working from home or in the field.
  • Noise level is low-to-moderate; depending on the location of community events.
  • Must be able to bend, reach, and lift a minimum of 40 lbs.

ABOUT YOU

Bachelor’s or Master’s degree in Special Education, Social Work, Psychology or a related field; 5 years' experience in a related field and 2 years of supervisory experience, preferably within a home and community setting. Experience in helping someone to expand their social capital and contribution to community.

Must be authentically committed to social justice for people with disabilities as well as a passion for seeing people to become active members of their communities. Should also be passionate about the well-being and empowerment of employees under their leadership.
