Seeking Case Managers for Treatment for Individuals Experiencing Homelessness (TIEH) initiative. Positions available in Summersville, Webster Springs, Maxwelton and/or Marlinton, WV. Case Managers administer evaluations, develop and implement individualized treatment strategies and provide targeted case management functions to assure individuals with serious mental illness (SMI), serious emotional disturbance (SED), or co-occurring disorder (COD) who are experiencing homelessness or at imminent risk of homelessness have access to needed services and resources. Provide supportive behavioral health counseling.
Bachelor’s Degree in a Human Service field required. West Virginia Social Work License preferred. Experience in behavioral health preferred. Valid driver’s license and vehicle required.
Must pass a Criminal Background Check and Employment Fitness Determination through WVCARES and a drug screen.
Demonstrated knowledge of basic computer skills required.
ESSENTIAL FUNCTIONS:
- Complete immediate screens for clients via a mobile crisis model and screen youth in schools.
- Partner with area schools to identify and obtain program referrals for families at risk of or experience homelessness; refer and follow-up on referrals.
- Ensure clients receive infectious disease screening and education.
- Ensure an ongoing formal and informal process to collect and interpret information about client strengths, needs, resources, and life goals to be used in the development of a person-centered service delivery plan.
- Administer evaluations, develop/implement service delivery plans.
- Provide intensive crisis services for high-risk individuals.
- Perform linkage/referral to all internal and external services and supports.
- Advocate on behalf of client to ensure continuity of services, system flexibility, integrated services, proper utilization of facilities and resources, and accessibility to services.
- Follow-up with client on a regular and consistent basis to ensure their needs (and goals) are being met and that circumstances and priorities have not changed and share information with other team members.
- Ensure adequate and appropriate crisis response planning procedures are available to the client and assist as necessary in accessing crisis support services and interventions.
- Continually evaluate the appropriateness of the treatment strategy and make appropriate modifications, establish new linkages, or engage in other dispositions as necessary.
- Provide supportive behavioral health counseling to maintain client progress towards identified goals and to assist in day-to-day management and problem solving as needed.
- Submit the appropriate required utilization management information to ensure all services are authorized prior to service. Follow-up on authorizations as needed.
- Assure quality service for clients by becoming knowledgeable of and adhering to rules, regulations and legal requirements and by documenting services provided in accordance with established standards.
- Secure information by adhering to HIPAA and 42 CFR Part 2 regulations and Seneca privacy policies and keeping client information confidential.
- Represent Seneca in a positive manner including maintaining a positive attitude and performing duties in a manner in accordance with Seneca’s Mission and Code of Conduct and Ethical Practices.
