RN-Care Consultant

PRN/Flexible Schedule/Remote/Contract Position

Senior Care At Home is seeking RNs to join us in our mission to help seniors live well at home. As a Care Consultant, you will assist clients and their families with decisions related to aging at home with Senior Care At Home services. You will carry out in-home assessments with prospective elderly clients and their family/authorized parties. The assessment evaluates the individual’s functionality, health, cognitive and emotional state, and living environment. The Care Consultant will develop a care plan to instruct in-home caregivers in the care of the client and will work with the Senior Care At Home care management team to assist with a successful client start of care.

Job functions include:

● perform in-home assessments

● secure written client consent for services

● develop care plans and set them up in software program (NO OASIS!)

● assist with coordinating start of care services, including caregiver selection and orientation

● be available by phone or in person for care consultation and reassessment visits as needed

● participate in electronic documentation of client service activity

Requirements:

● graduate of an accredited school of nursing with one year of clinical experience within the past five years or completion of an accredited refresher course within the past two years

● current Oklahoma RN license in good standing with the Oklahoma State Board of Nursing

● be flexible to meet with clients and families at a mutually agreed to time, including occasional evenings and weekends

● be comfortable in a sales/consultancy role to encourage the use of Senior Care At Home services as appropriate

● proficient in the use of computer software and gmail (and willing to learn new software)

● access to a computer and Internet from home is ideal. Senior Care At Home will provide office space with a computer if needed.

● ability to read, write, speak and understand English

● possess a valid driver's license and have use of an insured automobile

● clean criminal background

● minimum of 2 professional references.

This position is 1099-contract. This opportunity will be suited for someone who wants to add supplemental income and work a very flexible schedule. Senior Care At Home makes no guarantee on the number of hours to be worked or the number of assessments performed. Work opportunities are dependent on client demand.
