PACE is an alternative to nursing home care and – through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides – coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community.

Primary Purpose
Under the supervision of the Assistant Participant Care Manager, the PCA Department Coordinator will provide oversight for day-to-day Participant Care Delivery in the home and is responsible for the effective PCA daily workflow.
Duties and Responsibilities
Organize, plan, and delegate PCA daily tasks to include:
Participant personal care in the home.
Delivery of supplies in coordination with the IDT.
Provide coverage for all PCA tasks as needed and assist with resolving issues as they arise in coordination with the Home Care Scheduler.
Plan and assign PCA break and mealtimes
Order supplies for the department as needed
Contributes to and supports participant care in the home through close collaboration with all departments.
Assists prospective and newly enrolled participants become acclimated with P.A.C.E. and Center services.
Works closely with the Assistant Participant Care Manager to ensure participant safety, and satisfaction while receiving personal care in the home.
Troubleshoots issues in the field for PCA/CNA staff.
Ensures completion of care documentation.
Completes supervisory visits in the home to ensure participant quality of care.
Delivers start of care information to the participant or caregiver.
Will provide on-call support on evenings and weekends.
IDT/Other Duties and Responsibilities
Participate in and provide PCA representation at PCT/ IDT meetings (i.e., morning meeting, Care Plans).
Support the IDT by entering meeting notes into the EMR as needed or assigned.
Assist in developing and implementing written policies/procedures/protocols governing the operation of the department.
Responsible for managing the payroll system for direct reports.
Responsible for completing all required education and file requirements on time.
Documents in the participant’s medical record as required by policy.
Participates in Quality Improvement activities.
Must care, support and collaborate with everyone involved in the participant’s care including staff and the participant’s family using the participant centered care model.
Keeps leader informed of problems or issues.
Performs other duties as assigned.
Leader Responsibilities
This position has leadership responsibilities for the Participant Care Assistants in the Home Care setting or Day Center setting
Qualifications
High School Diploma or equivalent required, Associates Degree preferred.
A minimum of two years of professional leadership experience is required, or an equivalent combination of education and applicable experience.
Must be a Certified Nursing Assistant or complete PCA training provided by Senior Care Partners P.A.C.E.
Detail-oriented with good problem-solving skills.
Strong communication skills, both written and verbal.
Experience in managing and entering payroll information.
Demonstrates ability to effectively build team cohesion.
Well-developed ability to show discretion and maintain confidentiality.
Demonstrate ability to educate and empower others.
Strong MS Office Suite, Word, Outlook, PowerPoint, and Excel skills.
Minimum of one year of experience working with the frail, elderly, or long-term care population.

Possess current CPR certification by AHA or be willing and able to obtain it as offered by Senior Care Partners P.A.C.E.
Immunizations and vaccines must be current or medical documentation must be provided by a medical provider with reason(s) to request waiver. Required immunizations and vaccines include but Hepatitis B, TB, MMR, Varicella, TDaP, Flu, and COVID-19.
Must possess the ability to make independent decisions when circumstances warrant such action.
Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
Possess the ability to collaborate effectively with other IDT members.
Must have patience, tact, a cheerful disposition, and enthusiasm, as well as the willingness to handle difficult participants.
