Care Coordinators working for SeniorsPlus in the State of Maine assist clients to make informed choices to live safely and independently in their communities.
Support is provided primarily through telephonic care coordination, assisting clients to achieve maximum independence and to positively impact and improve their well-being. All of this occurs through a collaborative process of assessment, planning, implementing, and monitoring with other health care professionals to coordinate the client’s plan of care.
Some travel and home visits will be required post Covid.
REQUIREMENTS
- Bachelor’s degree in Health or Social Services
- Familiarity with the long term care system a plus
- Strong computer and organizational skills required
