What are the responsibilities and job description for the Integrated Care Manager-1 position at Sentara?
**THIS POSTION WILL COVER PRACTICES IN VIRGINIA BEACH, VA**
**ALL QUALIFIED CANDIDATES WILL RECEIVE A $10,000 SIGN-ON BONUS!**
Hours: FT (40 hours/wk) - Mon-Fri, Day Shift
HYBRID - will be working from the practice 5 days/week and then after 6 months, will have the opportunity to work 2 days per week remotely
NO NIGHTS, WEEKENDS, or HOLIDAYS
As an Integrated Care Manager with Sentara, you get to work together in an intensely collaborative way with the patients, family, and members of the medical team to provide patients with the resources they need and help prevent them from readmitting into the hospital. This position's primary duty is customizing what is best for the patients. That would include helping with things like home health, long-term care placement, home medical equipment, homeless and community resources. Qualifications:
- Active VA RN or Compact License
- ASN, BSN, or MSN
- 3 years case management experience
