Job Title: Resident Care Director

Department: Health &Wellness

FLSA Status: Full-time, Salary / Exempt

Reports To: Executive Director

Purpose:

To provide leadership and support to resident care services, ensuring that resident care is provided in a safe and effective manner, while upholding the principles of resident rights, compliance with state regulations, and in accordance with currently recognized nursing care standard and practices for Residential Care Facilities and Memory Care endorsements.

Minimum Eligibility Requirements:

  • Licensed Nurse (RN, LPN, LVN) with a strong background in long-term community-based care preferred
  • Bachelor’s degree preferred
  • State license current and in good standing
  • Current and valid state driver’s license and reliable transportation to conduct off-site assessments and networking
  • In-depth knowledge of current dementia-care practices and communication strategies
  • Previous supervisory and/or management experience in a related healthcare setting
  • Experience in training and team member development preferred
  • Ability to establish effective relationships with resident, family members and team members
  • Must be able to read, comprehend, write, and speak English to communicate effectively with residents, families, other team member and physicians
  • Must be able to communicate effectively with resident, families, team members, community and state officials, referral sources and the general public
  • Strong documentation skills
  • Must have basic computer skills, including Word and Excel
  • Ability to follow systems for monitoring resident health conditions and for ensuring efficient and safe delivery of resident care
  • Strong organizational skills and ability to re-prioritize daily tasks, in order to accommodate fluctuating needs of residents and the community
  • Ability to represent the community in a positive and professional manner
  • Must be able to meet all health-related requirements pertaining to state licensure
  • Must maintain current CPR/First Aid credentials
  • Must maintain current Food Handlers credentials
  • Must maintain annual Continuing Education credits
  • Must be able to perform duties and responsibilities (essential functions) with or without accommodation
  • Must have acceptable references
  • Subject to pre-employment screening including criminal background check, drug and TB testing, and physical

Essential Functions:

Management

  • Provides supervision for all care team members including hiring, training, evaluating and providing progressive discipline
  • Serves as member of the community management team and may assist Executive Director with other management duties as needed
  • Develop, post and monitor care department schedule and ensure adequate staffing on a daily basis for quality resident care
  • Operates department within established budgetary guidelines
  • Assists with marketing the community program and networks with referral sources within the senior housing and dementia-care communities at the direction of the Executive Director
  • Provides training to all community team members on nursing and dementia-related topics, and assists in maintaining team member training records
  • Maintains knowledge of current OSHA and state regulations, and routinely monitors community compliance with the regulations related to resident care and nursing functions

Program Development/Implementation

  • Ensures that the philosophy and concepts of the community’s program are promoted and adhered to on a consistent basis
  • Acts as a role model to all community team members in terms of professional appearance, conduct, resident communication and behavior management strategies
  • Ensures that each resident is routinely engaged in conversation, interaction, and activity that is consistent with life story, past interests, preferences and current ability level

Resident Care

  • Responsible for resident assessment and development of resident service plans that accurately reflect resident needs and are updated as needs change
  • Responsible for coordination and care department participation in resident and family care conferences
  • Responsible for communication between resident, family and physician regarding resident health care issues, changes in resident condition, and medication changes
  • Responsible for evaluations for changes in resident condition, monitoring documenting and implementing effective interventions
  • Performs skilled nursing tasks as defined by community policy and procedure, and as allowed by state law
  • Provides oversight of medication management systems, and ensures that medication are dispensed in a safe and effective manner, that eMARS are accurate and complete, and that medication cart is orderly, with all medication appropriately packaged and stored
  • Responsible for monitoring resident health concerns such as weight and nutrition, skin integrity, mobility, fall management, and pain management
  • Responsible for maintaining inventory of healthcare supplies and equipment, and follows appropriate communication protocol for purchasing
  • Reviews Resident Health Record on a routine basis and ensures that documentation meets community standards and adequately addresses the residents’ healthcare issues, including identified concerns, interventions and outcomes
  • Ensures that all resident care is provided in a safe and effective manner, while consistently maintaining resident dignity, choice, and respect
  • Give clinical direction, guidance, mentoring, and education to all team members
  • Adjust responsibilities as needed to ensure quality programming and care, with or without direct input from community Executive Director

  • Attends all required team member meetings and in-service training sessions
  • Upholds Residents’ Rights as defined by state regulations and by Lenity Management’ policy, at all times
  • Supports and upholds all other Lenity Management policies, and the service philosophy

Strength Level: Medium (M), according to the Classification of Jobs (COJ), involves the ability to exert 20 to 50 pounds of force occasionally, 10 to 25 pounds of force frequently, and greater than 10 pounds of force constantly, to move objects.

Stand: OCASIONAL to REQUENT standing intermittent with walking is performed for up to two hours at a time while making rounds and checking on residents. Floor surfaces are generally carpet, linoleum, tile or concrete.

Walk: OCCASIONAL to FREQUENT walking intermittent with standing is performed for up to two hours at a time while making rounds and checking on residents. Floor surfaces are generally carpet, linoleum, tile, vinyl, or concrete.

Sit: OCCASIONAL to FREQUENT sitting for up to two hours while charting activities and medical information by computer and/or hand, inputting payroll and communicating with families.

Lift: FREQUENT lift of 0-10 pounds for resident files and individual medical supplies. Lifting up to 25 pounds. SELDOM required when stocking medical supplies. Lifting up to 50 pounds is rarely (1-2 times per month) required to assist or transfer residents. The correct use of a transfer belt is required to avoid injury to the resident or nurse. Lifting is performed from floor to shoulder level. Lifting over shoulder level is SELDOM.

Carry: FREQUENT carrying up to 10 pounds when carrying resident treatment bag or other items. Carts are used for moving items weighing more than 10 pounds, such as medical equipment and supplies.

Push/Pull: OCCASIONAL push/pull cart with medical equipment and supplies requiring up to 10 pounds of force. SELDOM push/pull cart loaded with heavier supplies requiring push/pull force up to 25 pounds. Typically cart rolls easily over concrete walkways, carpeting and tile.

Push wheelchairs on a SELDOM basis. In regards to pushing wheelchairs the ratio is 8:1, so a 150 pound person in a wheelchair that weighs approximately 50 pounds would require 25 pounds of initial push force and then 20 pounds of kinetic force to maintain the pushing.

CPR is performed on a SELDOM basis – on average 2-4 times per year. The amount of push force required for CPR chest compression is estimated at 110 pounds maximum. This is a shared force for each upper extremity and the trunk flexion force application would be a shared event, thus each body segment would apply a maximum of approximately 33 pounds of force. CPR is administered until EMS arrives, typically 1-4 minutes.

Controls: OCCASIONAL intermittent use of various hand controls with knobs to turn, buttons to push and switches to flip as needed for blood pressure cuff and various other medical equipment. The use of foot controls is SELDOM.

Climb: Climbing stairs and ladders is not a requirement for this position.

Balance: Balance is within normal limits.

Crawl: Not a required physical demand of this position.

Bend/Stoop: FREQUENT bending required when providing treatment to residents, administering medications, taking vital signs and assisting seated residents. Must be able to perform CPR. Crouch, bend, stoop and kneel may often be used interchangeably at employee’s discretion.

Crouch: On a rare basis, may need to kneel or crouch to retrieve items at floor level. Must be able to perform CPR, crouch, bend, stoop and kneel may often be used interchangeably at employee’s discretion.

Twist: OCCASIONAL twisting at the waist may occur when providing medical treatments, assisting in transfer or when applying/changing daily bandages and leg wraps.

Kneel: On a rare basis, may need to kneel or crouch to retrieve items at floor level.

Handle/Grasp: CONSTANT grasping when handling pens/pencils for documenting information into medical charts, when handling paperwork and while using various medical supplies/equipment (stethoscope, thermometer, medications, syringes, etc.). SELDOM forceful grasp used when pushing resident in wheelchair. Grasping for transferring patients occurs on an infrequent monthly basis.

Fine Manipulation/Fingering: OCCASIONAL fine finger manipulation while using the computer and telephone and when performing first aid and administering shots.

Feeling: Must be able to take vitals (i.e. feel pulse) and must be able to detect temperature differences in skin.

Reach: Reaching OCCASIONALLY at all levels from standing and seated positions up to full arm extension when administering to residents who are bedridden and seated, reaching for supplies and using phone. Reaching above shoulder height is not necessary from standing position.

Vision: CONSTANT for doing job effectively and correctly. Normal or corrected to normal vision is required.

Talk/Hear: Necessary for communicating with others, safety awareness and for taking instructions from physicians and supervisor.

Taste/Smell: Smell is required and necessary to detect dangerous odors when providing wound treatment and performing health assessment.

Environmental Factors/Access: Work is performed primarily indoors. Walking between residential units and the office building will expose worker to weather condition. Will be exposed to household chemicals, human waste and bodily fluids, infection and viruses and/or sanitation chemicals. May use safety gloves for patient care, sanitation and laundry.

Work Environment Access: Wheelchair accessible. Restrooms, parking and break area provided.
