What are the responsibilities and job description for the Case Manager position at Seven Hills Foundation?
$4,000 sign-on bonus
The Case Manager is responsible for providing case management services to students and their families; initial and on-going collateral work with referral sources, guardians, courts, school systems, coordination of internal services that include quality assurance, conducting pre-placement interviews. The Case Manager will work as part of the treatment team and will participate, collaboratively, in an overall approach to treatment combining direct and indirect treatment work.
Provide professional presentation when representing the agency with external parties.
Ensures that team staff work within the Family Engagement model, and treat all family members, guardians, and visitors with dignity and respect.
Work collaboratively with funding agencies in the best interest of the student and community, and alerts appropriate administrator(s) in the identification of potential conflicts.
Ensures telephone and mail contact with family or other identified supportive adults in accordance with the individual service plan and program rules and standards.
Acts as a role model and reinforces pro-social behaviors with students and staff.
Promotes staff interventions that support the Stetson School’s ACORN approach.

Bachelor’s degree in related field of study
Minimum of two years human service experience preferred
Prior experience working with troubled adolescents and/or sexual offenders
Current Massachusetts Drivers License
