Registered Nurse Case Manager RN Case Manager
Overview:
We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our healthcare team. As a Registered Nurse Case Manager, you will play a crucial role in coordinating and providing comprehensive care for patients. This position requires strong clinical knowledge, excellent communication skills, and the ability to work collaboratively with a multidisciplinary team.
Duties:
- Conduct comprehensive assessments of patients' health status and develop individualized care plans
- Coordinate and monitor patient care throughout the healthcare continuum
- Collaborate with physicians, specialists, and other healthcare professionals to ensure optimal patient outcomes
- Provide patient education on disease management, treatment options, and preventive care
- Perform diagnostic evaluations and interpret test results
- Monitor vital signs and assess patients' physiological responses to treatment
- Assist with discharge planning and ensure smooth transitions of care
- Maintain accurate and up-to-date medical records
Experience:
- Bachelor's degree in Nursing (BSN) or equivalent
- NY state licensure as a Registered Nurse (RN)
- Minimum of 2 years of experience in acute care or hospital setting
- Experience in managed care or case management required
- Strong knowledge of physiology and medical terminology
- Excellent organizational and time management skills
- Effective communication and interpersonal skills
