Program Summary:

The Student Homelessness Initiative Partnership (SHIP) of Frederick County operates at the intersection of education, employment, and homelessness. Through partnerships with the broader community, SHIP aims to connect youth with the tools they need to reduce barriers to academic attainment, employment, and safe, stable housing.

Position Summary:

The Case Manager position will work directly within the New Horizons team to support the unique needs of youth experiencing homelessness. The Case Manager interfaces with youth, caretakers, school and service agency personnel to support progress of student service plans.

Expected Responsibilities:

  • Supports the daily operations of the New Horizons program
  • Gathers information to complete youth needs assessments and service plans with support and direction from the Program Administrator
  • Responsible for regular visits with program youth, tracks goals and progress
  • Maintains accurate and timely documentation
  • Acts as a mandated reporter
  • Acts as a liaison between academic, counseling, and outside services. Communicates regularly for purposes of case management
  • Conducts intake and housing assessments within the first 14 days of service
  • Provides a period of transition planning 30-60 days at the end of service
  • Administers Student Survey
  • Provides ongoing, flexible, youth driven case management services and follow up
  • Updates service plan every 90 days
  • Maintains client files in secure database
  • Conducts and reports on host home visits
  • Provides transportation or transport planning as needed
  • Provides some group facilitation
  • Collaborates with Frederick Community College and Hood College for purposes of case management and in support of College Horizons
  • Contributes to professional development by participating in continuing education, workshops and in-service training. Attends trauma focused training at minimum once every 60 days.
  • Complies with all organizational policies and procedures
  • Performs other related duties as assigned.

Necessary Skills and Qualifications:

  • Experience in case management, preferably working with a direct service provider
  • Ability to work legally in the United States
  • Excellent time management skills and ability to prioritize work
  • Strong communication and organizational skills, attention to detail
  • Ability to work both independently and as an integral part of the SHIP team
  • Comfort communicating with vulnerable families and youth, ability to say no to requests when necessary
  • Pass a criminal background check
  • Have a valid driver’s license, dependable automobile, and automobile insurance
  • Have a Bachelor’s degree in a related field or equivalent work experience
  • Skill in operation of modern office equipment such as personal computer (including MS Office), facsimile, copiers, scanners and telephones
  • Interpersonal skills necessary to develop and maintain effective and appropriate working relationships with clients, co-workers, and representatives of other agencies.
  • Ability to use sound judgment to detect unusual, harmful, or emergency situations and act accordingly
  • Ability to comprehend, process and apply both verbal and written skills appropriate to the job
  • Ability to consistently demonstrate sound ethics and judgment
  • Ability to maintain the confidentiality of information and professional boundaries

Preferred Qualifications:

  • Working knowledge of local agencies, community resources and organizations addressing the issues of youth, youth homelessness, and housing at large
  • Awareness of trauma, and willingness to learn and engage in trainings about childhood and adult trauma
  • Bilingual English/Spanish

Job Type: Full-time

Pay: $40,000.00 - $45,000.00 per year
