Job Summary
SCFG community health workers (CHW) promote health and wellness among individuals living with sickle cell in the state of Georgia to bridge the gap between health care access and community services and the realities of individuals’ lives. CHWs provide culturally appropriate services based on the need to improve health outcomes. Services are provided in community outreach settings, including homes, schools, hospitals, clinics, shelters, local businesses, and community centers.
Key Functions/Responsibilities
Provide community-based services reflecting the specific needs of individuals living with sickle cell disease and their families, including nutritional services and transportation assistance.
Identify and outreach to persons living with Sickle Cell Disease (SCD) who are out of care (have not seen a primary care physician for at least one year).
Establish trusting relationships with persons living with SCD and their families.
Establish and maintain relationships with clinics, hospitals, schools, urgent care centers, primary care physicians/providers, community health centers.
Conduct needs assessments to identify barriers to accessing health care for SCD and identify service needs.
Provide education, coaching, support, and follow-up to motivate persons living with SCD to be active, engaged participants in their health; effective management of SCD, and self-care.
Provide information and facilitate enrollment in the SCD patient-powered registry. Provide coaching and assistance as needed to complete the registry enrollment process and update as needed.
Provide ongoing follow-up to SCD clients via phone calls, home visits, and visits to other settings where persons can be found.
Provide consistent communication to the Health Educator to evaluate the status of persons living with SCD to ensure that reports and information provided clearly describe progress.
Be knowledgeable about community resources appropriate to the needs of persons living with SCD and their families.
Identify, generate, and maintain the database of community resources, services, and programs provided.
Work collaboratively and effectively with a team.
Collect, organize, and enter information about services provided and persons served in designated software.
Minimum Qualifications
High School Diploma or its equivalent, and at least one year of college
Good understanding of the community and services
Knowledge of Sickle Cell Disease and Sickle Cell Trait preferred
At least one year of experience working in a community-based setting
Strong computer skills (MS Office)
Excellent communication skills, such as listening well and using language appropriately. Well-developed written communication skills.
Ability and willingness to provide emotional support, encouragement, and motivation to persons living with SCD and their families.
Successful completion of the National Sickle Cell Disease Lay Program Certification Training
Valid driver’s license and reliable transportation
Have and maintain automobile liability insurance
Satisfactory driving record
Satisfactory criminal background record
Ability to follow directions
Ability to work a flexible schedule, including evenings and weekends
