Job summary:

To provide day and residential program management services to all the Consumers in the case load in accordance with the objectives in the Individual Service Agreement (ISA) for the Consumers, and all applicable regulations of the State of New Hampshire Division on Mental Health and Development Services.

Summary of essential job functions:

  • Provide program management oversight to an average caseload of 10-12 individuals which are subject to periodically change per the program management needs. In addition, provide program management oversight as a back-up program manager to caseload of 10-12 individuals to assist with coverage during transitions and absence/unavailability of the assigned program manager.
  • To ensure that an environment supportive of the Consumer’s rights and personal development is provided by the volunteer provider as defined in the Volunteer Adult Foster Care Agreement. An appropriate level of support and supervision is provided at all times per the Consumers Individual Service Agreement.
  • Participate actively and contribute in all consumer-centered conferences or team meetings and assist the volunteer provider in implementing Consumer’s residential and day goals. The Program Manager will be responsible for collecting daily notes and providing progress notes as required by the Agency. Specifically, the Program Manager will ensure that the Daily Notes are written as per weekly schedule and are focusing on the activities to support the day goal. Similarly, the monthly progress notes will focus on the update/progress on the goal and will be submitted to the Agency’s main office by at least the 7th of each month.
  • Where applicable the Program Manager will work with the behaviorist to implement and coordinate the presentation of the updated behavior plan to the Area Agency’s Human Rights Committee.
  • Perform documented monthly Consumer site visits (announced and unannounced) to review Consumer living environment, discussion on progress on goals & any concerns, review of client books to check, including but not limited to: fire drill, client monthly statement, auto and home insurance, attendance and reports. This will also include supervision on day program sites for Consumers to evaluate the quality of day services.
  • Work with the volunteer provider to ensure that the required reporting such as Daily Notes, Monthly Progress Notes, and Incident Reports etc. are done on a timely basis and sent in to the main office.
  • Maintain all client specific required documentation including but not limited to Annual Consent Form, Respite Consent Form, Health History Form, Health Information Release, Alone Time Assessment and Goal Worksheet etc. These documentations will be monitored and updated annually and as required.
  • To prepare and participate in the mandatory State site inspections. Respond to the non-compliance and concerns highlighted as part of the inspections and ensure that the corrective actions are implemented.
  • Familiarize with all the Agency and Area Agency policies to provide appropriate guidance to the volunteer providers and ensure compliance.
  • To ensure open communication with all team members and meet with the PMS at least once a week to update and discuss the Consumer’s progress. The PMS will be copied on all e-mail communication to the Consumer’s team members.
  • Program Manager will work with the company nurse to support the Consumer’s health care needs:
  • The Consumer’s annual physical is scheduled every year within the 12 months period.
  • Medication Order is not expired.
  • Medication Occurrences are documented and communicated.
  • Monitor Self Administration Assessment of Consumers to ensure that they are re-assessed annually.
  • Ensure corrective actions are taken on the QA Report by the volunteer providers.
  • Help trouble shoot any issues with the medical, psychiatric and dental appointments.
  • If required, attend all key medical appointments to ensure follow through of medical care.
  • Respond to any emergency situation, provide support to consumer and volunteer provider and follow the communication protocols.
  • To ensure that all required documentation such as criminal, certified motor vehicle checks, work authorization and registry checks for volunteer provider and respite are in place prior to them being authorized to work with a Consumer.
  • To ensure that all volunteer providers and respite staffs have completed all required Adult Foster Care training, and also complete all additional or supplemental Adult Foster Care training (client specific training) as may be recommended by the Agency or required by the state or federal government.
  • In the event that a Client Rights complaint is filed against the Volunteer Provider, Program Manager will work with the Agency to support the investigation and take all necessary actions as deemed necessary to ensure the safety and well-being of the Consumer.
  • To actively participate in cross training of new program managers to whom the program manager is transitioning Consumers from their caseload. This include but not limited to, Consumer support profile review, introduction to team, shadow in team meetings and state reviews, review of program related documentation etc.

Abilities required:

Proficient computer skills, skills in problem management, organizational skills, self development, planning, coaching, supervision, people management and team player.

Schedule:

Monday to Friday. In person with an option of Hybrid after 90 days probationary period.

Pay: $42,000.00 - $50,000.00 per year
