Overview:
The Population Health Clinical Coordinator is responsible for coordinating multiple aspects of care for patients with, chronic diseases, case management and care opportunities within the NP2U mobile Physician services. The Population Health Coordinator will build and manage high acuity patient telehealth monitoring, monitor resumption of care after hospitalization to reduce over all hospitalization of patients. The Population Health Clinical Coordinator assures that an integrated approach to the patients’ care coordination is fulfilled. This position is the liaison between providers, primary care clinics, specialist offices, patients, hospital systems, skilled buildings and freestanding care delivery centers or services for population health programs and related processes.
Responsibilities
• Prioritize & schedule coordination of patient’s chronic care needs.• Resource and contact for the providers to provide technical assistance to providers, clinical support staff in home health and hospice. Serve as a liaison and resource to the clinic staff and the patient for optimizing the coordination of care and immediate problem resolution. Collaborate with other team members to ensure the best patient outcomes.• Order laboratory tests or imaging exams per protocol to close care gaps.• Request and abstract clinical data (e.g. immunizations, laboratory tests, imaging studies, procedures) in Epic from external and/or other EHR sources.• Coordinate the Disease Management with Primary Care Provider scheduling processes for medical group patients in a centralized department setting.• Use scheduling systems and understand different protocols for coordinating services at multiple centers within specified timeframes.• Navigate data system(s) to research and/or record pertinent information in compliance with policies. Review patient chart to obtain documentation of requested services.• Receive and appropriately address phone calls from patients. Identify urgent or emergent situations and respond appropriately.• Utilize motivational and coaching techniques as necessary to remove barriers to care and engage patients in their care.• Use patient engagement tools such as, phone calls, letters, MyChart messages, and/or IVR solutions work with the healthcare team to identify and continuously improve the quality of services provided and actively collaborate in finding solutions.• Process all paperwork/forms related to laboratory scheduling.• Contact specialist offices to obtain clinical information (chart notes, diagnostic results) as needed to assist in disease management process.• Facilitate updating of patient registration information if inaccurate or no longer valid.
• Assist providers with appropriate ICD10 diagnosis coding as needed to comply with insurance coverage guidelines of various labs.• Supports mission, values and goals of Signature
Qualifications/Skillsets
EDUCATION and EXPERIENCE:
• Preferred two years recent (within the past five years) experience in acute, long-term care or home care/hospice.• RN Degree from an Accredited Nursing Educational Institution• RN current License to practice in this State• Excellent verbal, written and computer literacy skills in Outlook, Word and Excel at a minimum• Direct patient care as needed• Ability to maintain assigned work hours: Has sufficient endurance to perform tasks over long periods of work hours• Ability to travel to all business and patient locations
QUALIFICATIONS and SKILLS:
• Knowledge of medical terminology, disease processes, medical decision making, and prior experience in utilizing disease registries or similar databases.• Excellent verbal communication and interpersonal skills, with preferred experience in motivational interviewing / coaching.• Experience with electronic medical records and/or computerized billing systems.• Epic or other Electronic Health Record /care management software experience.• Computer skills including Microsoft Windows and Office Applications (Word, Access, and Excel).• Excellent typing skills (60 words/minute), computer keyboard, and mouse skills for expedited data processing.
