Are you an RN with a passion for hospice care? We are searching for a compassionate RN Case Manager to provide top-notch care at an excellent facility in Carlsbad, CA!
You will join a passionate and motivated team who pairs compassion and heartfelt care with clinical and hospice expertise. This is a small, 6-bed facility that provides hospice care both for patients on-site and for patients residing in their homes within a small radius of the facility.

Hourly Rate: $45-55/hour
- Full-Time Schedule (40 hours/wk): Monday - Friday, 8:30-5pm, with one Saturday OR Sunday every 6 weeks (RN Case Manager will trade one weekday with a weekend day to maintain a 40 hours/wk schedule)
As the RN Case Manager, you will....

Drive to patient homes (Patient Load: 16-18/ ~20 visits/wk / avg. 4/day) to assess and administer care to patients (mileage reimbursed for all home care visits)

Follow the hospice care plan, provide skilled and palliative care and coordinate the services provided to your patient(s)

Cooperate with the attending physician, facilitys Medical Director, and other members of the interdisciplinary hospice team
- Manage an average patient load of 16-18/visits/wk
Requirements

Active, unencumbered RN license to practice in the State of California

CPR certification
- Preferred: 1 year of RN experience
