Job Summary: The care partner performs a variety of functions under the general supervision of the Director to promote the general health and welfare of residents in need of some aspects of assisted living. Adherence to departmental standards is required. The care partner is responsible for providing direct care to and support of clients in a private duty setting.
Typical Job Duties (based on client need and care plan)
Performs duties as assigned on the Plan of Care in the client’s home.
- Assist clients with Activities of Daily Living not limited to Bathing, Dressing, Toileting, Grooming, Oral hygiene.
- Medication Reminders
- Ambulation
- Meal preparation
- Provide for a clean, safe, healthy environment for client and family members.
- Provide light housekeeping which includes laundry and changing of bed linens.
- Assist in meeting the clients’ nutritional needs by assisting with meal preparation, grocery shopping, dishes, and kitchen clean-up.
- Remind client to take self-administered medications.
- Observe and report any changes in client’s mental, physical, or emotional condition to Director or immediate supervisor in a timely manner.
- Establish and maintain effective communication and a professional relationship with client, family members and coworkers.
- Participate in monthly in-service/staff meetings and annual continuing education programs.
- Complete required documentation of services delivered in client’s Care Binder.
- Maintain confidentiality regarding client information
- Other reasonable related duties as assigned.
- Provide companionship including but not limited to
- Talking and listening
- Reading aloud
- Providing social and emotional support
- Promote the client’s mental alertness through involvement in activities of interest
- Assist with Instrumental Activities of Daily Living that may include but not limited to
- Assisting with telephone calls
- Letter writing
- Accompanying client on walks, community trips, doctor office visits, beauty salon etc.
Position requires flexibility including weekends and holidays
Qualifications
Essential Physical and Sensory Requirements:

- Must be able to stand, sit and walk for extended periods throughout the working day
- Must be able to maintain balance, kneel, crouch, grasp, bend, pull and reach for extended periods throughout the working day
- Must be able to perform repetitive motions for extended periods throughout the working day
- Must be able to lift, push and/or pull a minimum of 50 pounds

Qualifications
- US High School Diploma or Equivalent

- Previous experience in related field

- Attends all required in-service training.
- Follows uniform and dress codes including attention to personal hygiene. Wears ID badge at all times while on duty.
