Overview:
The Nursing Care Coordinator, Pulmonary Hypertension Center (PHC)
performs the primary function of a professional nurse leader in planning,
organizing, coordinating, directing, and evaluating the nursing care of all
patients in the Pulmonary Hypertension Center, with the goal of assisting
patients to attain, maintain, and restore health when possible. They are the
liaison between the patient, physician, and other components of patient care.
In this role, the Nursing Care Coordinator provides patient care
responsibilities within the RN scope of practice. They will also assist the patient with health
education regarding their disease process, medication refills, and general
questions about their care. The Nursing Care Coordinator, PHC must have a
strong working knowledge of patient hemodynamic management and pulmonary
hypertension disease process. They must also possess excellent computer
skills and knowledge. Expectation is
for all performed duties to be in accordance with Singing River Health System
procedures and policies, accreditation organization, and governing guidance
and publications for health care employees

Education:
Graduate from an accredited School of
Nursing required; BSN preferred.
License:
Current Mississippi State licensure to practice as a Nurse.
Certifications:
BCLS and ACLS
certification required.
Experience:
Minimum of five (5) years experience in clinical areas with two
(2) years critical care experience. Extensive knowledge of the Cardiovascular
and Pulmonary System and hemodynamic management.
Reports to:
Patient Care Manager, Heart & Vascular Services
Supervises:
None
Physical Demands:
Stands, turns, bends, stoops, lifts,
climbs, and uses many physical motions in performing day-to-day activities.
Auditory acuity to hear patient
verbal communication and gather data using auscultatory skills. Manual
dexterity of hands and feet to ambulate 6-7 1/2 hours per 8 hour shift and
adroitly manage multiple pieces of equipment/tubes/invasive lines.
Must be able to stand for extended
periods of time and do a great deal of walking without experiencing undue
fatigue. Must have high energy level and be capable of handling high pressure
situations. May be exposed to communicable disease or radiation.
Mental Demands:
Keen mental faculties/assessment and
decision making skills in the management of acute and chronically ill
patients. Communication/speaking/enunciation skills to receive and give
information in person and by telephone. Emotional stability conducive to
dealing with high stress level for 6-8 hours/8 hour shift associated with
care of acute patient/family, rapidly changing patient conditions, emotional
demands of patients and families, and demands of maintaining effective
working relationships with peers, managers, physicians, and other health care
workers.
Special Demands:
This position is a SRHS Pulmonary/Critical Care
Service Line.
This individual will be expected to cross train and work in multiple facilities and service line
departments when appropriate. Job requires traveling throughout the SRHS
service area.
Must have valid
driver license, liability insurance of personal vehicle used in SRHS
activity, and a satisfactory driving record.
Must possess superior customer service skills and
professional etiquette. Must possess
proficient knowledge and ability to use a computer (must be keyboard
proficient) and other office technology (i.e., telephone, fax, etc.), MS
Outlook and Word.
