Job Summary: The Assessment RN is a will be responsible for Admissions & Assessments and receive a flat rate per assessment. The Assessment RN can choose his/her location willing to travel within designated service area open vacancy. The Assessment RN willprovide information for the evaluations of nursing assistants, PCAs, HHAs, Companions and LPN’s concerning performance appraisals and in educating, consoling, or listening to families and clients. The Assessment RN will evaluate patient’s condition, completing appropriate clinical documentation under the direction of the Nursing Supervisor. The Assessment RN will implement and update the client’s plan as needed in order to ensure quality care is being implemented appropriately.ning performance appraisals and in educating, consoling, or listening to families and clients.
Work Hours: This position is flexible and aligned with the client’s availability.
Duties: The following list of duties is a guideline of possible duties to be performed independently or as a group of duties in the course of working with the client. The RN duties are not limited to those listed.
1. Is aware of areas that make residents subject to abuse and prevents such abuse.
2. Complete assessments, load data into online assessment tool and train applicable caregiver.
3. Maintains open lines of communication among the nursing shifts, and departments, supervisors, and families.
4. Is aware of safety issues and strives to accomplish all responsibilities in a safe manner, consistently reinforces good safety attitudes and practices displayed by other staff members.
5. Controls, prepares, and administer treatments and medications with knowledge of purpose and adverse effects of the nursing care provided.
6. Functions as a knowledgeable and supportive committee member upon request by the Director of Nursing to maintain high levels of health care.
7. Complete all documentation for assessments, observations, planning evaluation, orders by physicians and care provided to clients.
8. Utilize professional assessment skills to identify physical, mental, and psychosocial needs of residents, patients and is able to develop a care plan to meet these needs in a multi-disciplinary approach.
9. Functions in a supervisor capacity as assigned for the completion of a residents and patients care, employee’s guidelines and disciplines, and maintenance of a positive environment.
10. Functions as a resource person and mentor to peer and subordinates to maintain professional nursing standards.
11. Acts on clients behalf to assure patient rights and is aware of and prevents residents’ abuse.
Qualifications:
1. Current certificate in standard first aid and CPR.
2. Current TB test
3. Current RN license in this state, or permit to practice. Or NP needs, a master’s degree and Nurse Practitioner certification
4. Able to supervise (instruct, assign work, etc) Nursing Assistants, HHAs, PCAs, Companions.
5. Valid driver’s licence and reliable transportation
6. Ability to work independently.
7. Excellent communication and listening skills.
8. Compassionate, patient and caring nature.
9. Able to work in a stressful environment.
10. Must be physically and mentally fit to handle the demands of care giving.
