Specialized Habilitation Specialist
Location: Hiring for 1 position in Pierce County and 1 position in Lewis County
Wage: $40/hr.
Shift: Day shift. Monday-Friday.
Part Time
Start Date: When filled.

Habilitation Specialist: You will provide Specialized Habilitation services or/and Staff/Family Consultation services.
Specialized Habilitation Service: Provide community-based and individualized support with the intent of helping the client reach their habilitative goals. Specialized Habilitation services must promote inclusion in the community and be individualized to the client participant. Supports shall be provided with the intent of reaching a habilitative goal identified in the PCSP and in compliance with the Specialized Habilitation Policy. This is not a clinical position. You will teach the client life skills such as self-advocating, building confidence and self-esteem, social communication skills, upholding one’s values, and coping strategies.
Roles and Responsibilities:
1) Will provide support to address up to three goals.
2) Implement a written Initial Specialized Habilitation plan using the form provided. The Initial Specialized Habilitation Plan must include no more than three goals at a time. Employees must follow the guidelines of the initial plan.
3) Complete a quarterly report every 90 days of service. The report must describe the client’s progress using the form provided.
4) Initiate a referral to a physician, mental health professional or other appropriate resource if a client presents with a potential underlying medical, mental health, or educational support needs.
5) May take a shuttle with client to activities outside the assistant living facility or home.
Staff and Family Consultation Service: Consultation support is provided directly to a client’s family or direct service providers to meet the client’s specific needs. Must create an initial plan of work with the client and family, staff, and provider. The initial plan of work must be created using the DDA-provided template, and indicate how they plan to reach the client’s goals.
Requirements:
1. A bachelor’s or master’s degree in psychology, sociology, social work, education, child development, nursing, or gerontology. A certified life coach certificate is accepted. A related field and experience may be substituted.
2. Minimum of 2 years or more working and teaching life skills to adults with developmental disabilities.
3. Have experience teaching caregiver’s skills to help their client/child. This includes behavioral challenges, coping skills for in and out of the home, self-advocating skills, and other skills or methods that are needed.
4. Must be able to work in the United States.
5. Must be vaccinated for Covid-19 and show proof of vaccination.
6. Be able to push a client in a wheelchair. Accommodations can be made if needed.
7. Must be able to pass a background check.
8. Must be able to travel to at least two different locations a day to provide services.
Note: Clients come and go. Hours can fluctuate.
