The Case Manager will work closely with healthcare providers to help clients achieve their recovery goals.
Responsibilities
- Perform intake assessments
- Create individualized treatment plans for each client
- Maintain detailed and accurate documentation of patient information and treatment plan
- Maintain accurate, timely, and confidential client records in compliance with agency policies, as well as local, state, and federal regulations (e.g., HIPAA).
Prepare progress notes, treatment summaries, and other required documentation for clinical reviews and case management meetings.
