Job Summary:
Provide individualized Housing Support to individuals. This candidate will be passionate about helping individuals in need of stable housing. The Housing Manager will play a vital role in ensuring tenants have access to safe, affordable housing while supporting their unique challenges. Responsibilities:
- Attends work regularly and in a timely manner
- Always represent the company in a professional manner
- Self-motivated and able to work one-on-one with individuals with limited abilities
- Building positive professional relationships with individuals served and identify strengths and interests
- Advocate for person needs and resources within our company framework and communities
- Monitor person’s progress toward established service goals
- Develops an individualized person-centered transition plan
- Works with waiver Case Managers to arrange details and provide weekly updates
- Call clients with case updates twice weekly
- Conduct one-on-one visits with the person at locations including their homes and offices
- Transport clients with lack of transportation access to complete housing requirements
- Assistance completing rental applications, lease agreements, and meeting with landlords
- Help determining eligibility for economic support and research affordable housing
- Assesses and refers individuals to appropriate community services and resources
- Support and participate with packing items and moving belongings to new locations
- Support and assistance in accessing furniture and other household needs
- Secure and inquire moving assistance and vouchers: “We focus on people, not programs”
- Teach individuals the skills needed to live in the community
- Complete assessments of individual and family strengths and needs to develop, implement, and maintain service plans
- Maintain and complete, accurate, and up-to-date client records to effectively document case activity and support case decisions
- Provide coordination services by assessing, planning, and authorizing provisions of services in accordance with respective laws, rules, and company policies

Pay: $22.00 - $27.00 per hour
