MDS Coordinator
Full-time – 32-40 hours per week
South Kingstown Nursing & Rehab, a 96-bed skilled nursing care, dementia and sub-acute facility located in West Kingston, RI has an exciting opportunity for a candidate with the following qualifications.
Responsibilities include:
- Coordinate and complete MDS assessments and care plans for assigned residents.
- Conduct care plan conferences for assigned residents.
- Maintain and update care plans.
- Participate in facility meetings as requested.
- Monitor and audit clinical documentation to ensure accuracy.
Qualifications:
- Licensed in the state of Rhode Island as an RN or LPN with LTC experience.
- Basic knowledge of MDS and careplan process.
- Minimum of one year of MDS experience in a long-term care facility.
- Should possess the ability to be accurate, concise, detail-oriented and able to work cooperatively as a member of a team.
- Should possess the ability to organize, prioritize, work in a time-sensitive environment, and communicate within the organization.
- Should have excellent assessment and documentation skills.
- Prior case management skills a plus.
