MDS Coordinator
Full-time – 32-40 hours per week
South Kingstown Nursing & Rehab, a 96-bed skilled nursing care, dementia and sub-acute facility located in West Kingston, RI has an exciting opportunity for a candidate with the following qualifications.
Responsibilities include:
Coordinate and complete MDS assessments and care plans for assigned residents.
Conduct care plan conferences for assigned residents.
Maintain and update care plans.
Participate in facility meetings as requested.
Monitor and audit clinical documentation to ensure accuracy.
Qualifications:
Licensed in the state of Rhode Island as an RN or LPN with LTC experience.
Basic knowledge of MDS and careplan process.
Minimum of one year of MDS experience in a long-term care facility.
Should possess the ability to be accurate, concise, detail-oriented and able to work cooperatively as a member of a team.
Should possess the ability to organize, prioritize, work in a time-sensitive environment, and communicate within the organization.
Should have excellent assessment and documentation skills.
