FNP

Position based in La Junta, Colorado
FT/40, FT/30 (benefit eligible) or PRN

Requirements:
- Master's Degree
- Colorado/Transferable APN (board certified or possession of waiver), DEA, and, if applicable, MAT Waiver
- Ability to pass drug test and background check (including MVR)
- TB test result (within past 12 months)
- COVID vaccination or approved exemption
- Ability and willingness to work a varied schedule including holidays, evenings and weekends
- Travel to branch offices upon need
- English (speaking, writing and reading) and basic computer skills
- Timely and proper attendance
- Appropriate and effective work performance
- Ability to take direction and follow instruction
- May require:
- Valid Colorado driver's license, ability to drive, insurability and willingness to travel
- Annual flu vaccination
Main Duties:
- Perform all services in accordance with generally recognized professional standards and practices of the profession and to the reasonable satisfaction of SHG.
- Exhibit a thorough knowledge of the theories, principles, practices, and techniques of medicine and behavioral health.
- Comply with all applicable regulations.
- Provide appropriate and timely patient care at all times.
- Diagnose and treat patient conditions, properly utilizing agency processes and guidelines.
- Investigate, develop, and implement treatment protocols specific to pharmacotherapy.
- Manage common acute/chronic conditions and health care maintenance for all patients.
- Initiate and respond to coordination of care activities on behalf of patients with primary care providers, psychiatrists, and other healthcare personnel.
- Process medical prescription and refill requests, lab results, and general coordination of care for all patients.
- Be proficient and timely in documentation, monitoring patient charts, and other reporting data.
- Offer or refer to medication-assisted treatment (MAT) program, as appropriate and as directed.
- Provide medical services to acute treatment unit (ATU), as directed.
- Provide or refer to behavioral health groups with a focus on patient needs, such as improving health status, medication management, nutrition, and chronic disease care, as appropriate and as directed.
Salary
- $86,000-$109,686/Annualized wage (FT/40-exempt). FT/30 and PRN will be at prorated rate.
