Dialysis Social Worker

We are looking for a compassionate Social Worker to be part of our interdisciplinary team. Our mission is to provide dialysis services in a manner that achieves clinical excellence, patient satisfaction, and cost effectiveness. We are looking for a professional that wants to be a strong patient advocate, and help foster the patients coping mechanisms in order to improve and better their daily lives and experiences.

This position requires travel to five facilities located in Mt. Pleasant, West Burlington, Ft. Madison, Keokuk, and Fairfield, IA. Schedule is flexible and drive time and mileage is paid. Main location is negotiable, but travel is neccessary in order to complete a monthly face to face visit with all patients.

Responsibilities

• Provides on-going comprehensive psychosocial assessment and counseling services to chronic kidney disease patients and their families to improve their quality of life. Montly face to face interactions with patients in order to fully comprehend the patient's needs.
• Provides case management as it focuses on both the patient’s psychosocial status and the status of the larger social system. It integrates counseling or direct psychotherapeutic practice and coordination of services of others. The goal is to optimize patient functioning.
• Refers patients and families to a wide range of community services, including providers of health care coverage (Medicare, secondary insurance options, Medicaid, Waiver services), income maintenance programs (DHS, SSA-disability determination process),transportation services, vocational rehabilitation services, exercise programs, long term care facilities, volunteer opportunities, local, State, Federal and private agencies (pharmaceutical assistance, AKF, NKF): etc and helps patients and families use these services.
• Coordinates care with hospitals and long term care facilities.
• Participates in dialysis related meetings including short and long term patient care conferences, family meetings and Quality Assurance.
• Advocacy on patient’s behalf within the setting and with appropriate local, state and federal agencies and programs, and development of services.
• Addresses end of life issues with patients, families and the multidisciplinary treatment team including advance directives, DNR orders, and discontinuation of dialysis and hospice referrals.
• Participates in patient, family and staff orientation and education by developing educational materials and providing in-service programs for staff as requested.
• Maintains social work documentation in patient’s medical record. Documentation should show implementation of appropriate treatment modalities which may include, but not limited to: Maslow’s Hierarchy of Needs, Solution Focused Approach, Task Centered Approach, Crisis Intervention, Problem Solving, and Supportive Counseling.
• Administer KDQOL and load information into software for evaluation. Address findings that are below standard with IDT.
• Remain up to date on research, literature and developments in Renal Social Work.

Job Type: Full-time

Pay: Up to $30.00 per hour
