Overview:
The COMAT Case Manager assesses and screens the consumer determining needs and appropriate levels of service. He or she completes clinical evaluations/assessments, assists in service planning, and initiates direct services for consumers entering outpatient program. He or she provides case management services to assist consumers in their recovery.
Note: This job description is not intended to be all-inclusive. Employee may perform other related duties as assigned to meet the ongoing needs of the organization.
Qualifications:
- Bachelor’s Degree in a Human Service Field
- One (1) year experience in working with consumers with mental illness and/or addiction
- Valid driver’s license and satisfactory driving record
Job Type: Full-time
Pay: From $22.79 per hour
