Overview:
Case Managers provide a variety of services to consumers who are experiencing mental health and/or substance use challenges, with the overall goal of ensuring consumers receive appropriate care and have access to resources that promote their wellbeing.
Note: This job description is not intended to be all-inclusive. Employee may perform other related duties as assigned to meet the ongoing needs of the organization.
Qualifications:
- Bachelor’s degree in social work, counseling, or a related field
- One (1) year experience in working with specific population to be served
- Related license and/or certification
- Valid driver’s license and satisfactory driving record
