$50,532.00/yr minimum - $75,798.00/yr maximum (range increases based on education, experience and certifications)
Responsible for the overall operation of social service activities within the emergency department.
Responsibilities
Role Specific Responsibilities
· Complete initial and ongoing psychiatric assessments for identified
· atients who present with mental health issues in the Emergency Department.
· Through the use of high risk screens and communication with ED providers, determine and refer appropriate cases for admission, transfer to psychiatric hospital, or community crisis support as it relates to discharge from the Emergency Department.
· Identify indicators of abuse or neglect and intervene per regulatory requirements.
· Coordinate discharge plan in conjunction with case manager, patient, family/significant others.
· Present options to patient, family/significant others to address their needs and assist them with decision-making process.
· Coordinate information and referrals for financial and community resources in conjunction with case manager.
· Coordinate patient care with inpatient/referral placement and communicate issues assessed and possible interventions.
· Maintain confidentiality, be courteous to all patients, families and all other hospital staff-addresses and follow up on departmental patient concerns.
· Provide excellent customer services. Know and practice hospital identified customer service behaviors. Identify responsiveness to customer needs as a personal priority.
· Support the importance of continuous quality improvement. Participate in department quality improvement projects. Participate in hospital wide quality improvement projects, as needed.
· Support Service Excellence initiatives contributing to the organization becoming a top performer in quality care, patient safety and patient
satisfaction strategies: Understand and apply techniques to support the provision of population based appropriate care utilizing Joint Commission standards, CMS Core Measure requirements, and other best practices.
· Be a self-learner and foster learning in others. Pursue continuing education in area of expertise/leadership. Achieve and maintain hospital wide and department identified competencies.
· Participate in educational offerings provided by the hospital; attend all mandatory training programs and actively participate in departmental staff meetings.

Qualifications
Education
· Master’s Degree in Social Work
Licenses and Certification
· LCSW
Experience and Skills
· Technical Experience: Minimum two years of clinical and/or medical social work experience is required. Master’s Degree from a School of Social Work accredited by the Council of Social Work Education.
· Knowledge of contemporary behavioral and systems theories relevant to health care; experience in crisis intervention; addiction disorders/cross addictions; excellent knowledge of the concepts of stigma, addiction, substance abuse identification and interventions, recovery and relapse; support for victims of abuse, neglect or violence
